Medical Records Requests
Medical record procedures are regulated by the HIPAA Notice of Privacy Practices. Your records are completely confidential. Due to HIPAA regulations, your medical information can be released only after your written authorization is received.
Print the preferred PDF form and return it to us.

Office Visit Policies
To schedule an appointment, please call (248) 647-9860, prompt 3.
We know that some issues can’t wait. Generations OB-GYN offers same-day appointments whenever possible, especially for urgent or time-sensitive concerns. If you need to be seen quickly, please call our office, and we’ll do our best to accommodate you.
Mutual Respect of Time
- We strive to provide excellent medical care to our patients and to stay on schedule; however, emergencies do arise.
- We ask that you arrive 15 minutes before your scheduled appointment time and complete your pre-registration paperwork ahead of time. This allows the provider and patient service staff to best prepare for your visit.
- If you need to cancel or reschedule your appointment, please call the office as soon as possible.
Cancellations
Appointments not cancelled with at least a 24-hour notice will result in a $50 patient charge. Additionally, all hospital and in-office surgical appointments not properly cancelled are charged $250. Thank you for your understanding.
New Patients
When you schedule your first appointment, you will receive a set of new patient forms via email. Please pre-register before your appointment to save both patient and provider time.
To ensure the best quality of care, it is very important to have previous medical or prenatal records sent to our office prior to your scheduled appointment.
Telemedicine
For your convenience, we offer virtual Telemedicine appointments when appropriate. They can be used to offer a treatment plan (including prescriptions, if needed) or to evaluate the need for an in-person visit.
Telemedicine appointments will be billed to your insurance, similar to an office visit.
Insurance & Billing
We are committed to providing you with the highest caliber of care. As part of your relationship with Generations Ob/Gyn, it is important that you have a clear understanding of our financial policies, as well as your financial responsibilities.
It is your responsibility to know the limits and coverage of your particular health insurance policy. Our billing staff will do their best to assist you with insurance questions; however, if you have questions about your coverage, it is best to check with your specific insurance company.
Insurances Accepted

Copays/Deductibles
Your plan may require that you pay a co-payment, a deductible, or part of the bill (co-insurance). If you have any questions about your financial responsibility or benefits, please call the Customer Service number on your medical insurance card.
- Co-payments are due at the time of your office visit.
- Please bring your insurance card to each visit.
Payments
Financial statements are sent via email to the address we have on file in your patient chart. Payments are due upon receipt.

Financial Policies
Before you make your appointment, please review the Generations OB-GYN Authorization and Financial Responsibility Agreement Policy. Obstetric patients should also review the Generations OB-GYN Maternity/Obstetric Financial Responsibility Agreement Policy. You will receive a pre-registration email that includes this information and requires your electronic signature.
Hospital and Laboratory Charges
Corewell Health bills insurance separately from our practice. For any questions regarding hospital charges, facility fees, or laboratory charges, please reach out to the hospital’s billing department directly by calling (833) 261-4563 or visit Corewell Health’s billing page.

Mammography & Blood Draw Lab Services
We partner with our affiliate Corewell Health Beaumont Hospital to provide you with convenient digital mammography services and on-site blood draw lab services.
It is your responsibility to contact your insurance company to confirm that these services are a covered benefit under your policy.
Mammography
Please use the search tool to find a location that works well for you. Hours vary by location, and an appointment is required.

- Royal Oak Hosptial Imaging Center: 3581 W. 13 Mild Road Floor 2, Royal Oak, MI 48073
Blood Draw Lab Services
- We can quickly complete lab testing at the time of your visit, in a comfortable and familiar setting, without the need for an extra trip to the lab.
- If you need to have your labs drawn on a different day than your visit, there are also several Corewell Health laboratories across metro Detroit. Hours vary by location.
Prescription Refill Requests
- Contact our prescription line at (248) 647-9860, ext 303.
- Please allow 48 hours for processing. Prescriptions will be sent to the preferred pharmacy on file with the office unless otherwise requested.
- If this request is for a new or changed prescription, it must first be reviewed by your healthcare provider prior to approval. Please allow for additional processing time.
- It is important to carry a list of your medications. You can print this medication/allergy inventory form, if it is helpful.

If you cannot reach our office and you have a medical emergency or are in labor, please call our answering service at (248) 584-6142. This is reserved for medical emergencies and labor questions only.

Our physicians have privileges at Corewell Health Beaumont Hospital, Royal Oak Campus.
Confidentiality
Our confidentiality procedures are regulated by the Health Insurance portability and Accountability Act (HIPAA). We comply completely with these guidelines. All of your medical information is kept strictly confidential.
